Thank you for taking the time to speak to me,

My name is Marie Azzi  and I am a NDIS provider from Perpetual Care

Our vision is optimising social and economic independence and full participation for people with disability. An important part of our vision is striving to become the number one preferred service provider in our community.

I am writing to express my interest in working with you and your current participants requiring a NDIS Provider, the services I provide are:

• 0106- Assisting with Coordination
• 0108-Assisting with transport
• 0120- light household duties
• 0125-Participation in the community

My current qualifications and experience include **Management/law degree**

I am located in **Mount Lewis** and am able to take on any self or plan managed participants within **Sydney area**. My company is home based although we are happy to travel to the participants to provide services.

I am currently waiting on my  registration number for the NDIS Commission.

Our operating hours are 24/7 and are available to start taking on new participants from the 15/01/2023 onwards.
